Your Community Health is participating in the Voice of the Patient GP Survey. It helps us understand your experience with our medical clinic and how we can keep improving this service.
The Survey runs from 31 July -11 August and is run by Insync Surveys, an independent contractor.
Completing the Survey
If you have attended an appointment at our East Reservoir medical clinic or through telehealth the last three months, you will receive a text message from us inviting you to participate in the online survey. If you attend our medical clinic in person during the survey dates, we will ask if you would like to complete the survey while you wait for your appointment.
A few important things to know about the survey:
- Participating in the survey is voluntary.
- Your responses are anonymous, tell us how the East Reservoir medical clinic is doing and helps us improve our service.
- The Survey is available in:
- English
- Arabic
- Chinese
- Greek
- Italian
- Vietnamese.
Protecting your privacy
Your privacy is important to us and we protects the privacy and confidentiality of all clients and complies with privacy legislation.
Your contact details have not and will not be shared with Insync Surveys, as all responses are anonymous and confidential.

Opting out
If you do not want to complete the Survey, you may ignore the text message with the survey link or decline to fill out the survey when at our East Reservoir medical clinic. Nothing further is required to opt out of the survey.
