We are participating in the 2025 Community Health First Survey and want to hear from you about your experience receiving health and wellbeing services from us over the past year.

Your feedback is important, it helps us understand what is working well and how we can improve our services to better meet community needs.

Community Health First brings together all 22 of Victoria’s registered, independent health services to improve the health, wellbeing and quality of life for all Victorians.

About the survey

The survey gives clients and carers the chance to share their experiences and views about the care and support they have received. The information collected will help build a picture of the difference community health services are making across Victoria, and identify areas for improvement.

Key details

  • Takes approximately 5 minutes to complete
  • Responses are confidential and anonymous
  • Survey closes on 31 August 2025
  • Participants can choose to enter a draw to win a $50 voucher as a thank you for their time

Your responses will be combined with feedback from people across Victoria to demonstrate the collective impact of these services. This information will help guide improvements both locally and across the state and may be featured in publications such as the Community Health First impact report.

In person – visit our centre to complete a paper copy of the survey

Online – scan the QR code to complete the survey on your device
