Health Independence Program

This program helps adults who have attended, or may attend, the Austin Health Emergency department.

What it is HIP?

The Health Independence Program (HIP) aims to help people access any services they need to stay safe and independent for as long possible in their own home. People can be assisted in a variety of ways. It is available in the Austin Health catchment area.

Who can use HIP?

HIP is available to:

  • people over 65 years or younger people with an ongoing disability
  • people who require extra support to manage at home
  • people living in Banyule, Boroondara, Darebin, Hume, Manningham, Moreland, Nillumbik and Whittlesea.
  • Cost

  • Care management is free, but clients may be asked to contribute towards the cost of external services.

What services are included?

Services vary depending on clients’ needs. This can include:

  • carer support
  • personal care
  • respite care, either in-home or facility based
  • allied health services
  • communication support with GPs and specialists
  • referral for aged care assessments
  • pharmacy review
  • NDIS application support
  • referral to other services and agencies.

A referral includes client details, GP details, outline of their condition, circumstance and reason for referral.
