
We run a variety of social groups each week, plus a number of volunteer of community led social groups run from our health centres.
Your Community Health Social Support Program provides out-of-home activities and support to older people, and people with a disability and their carers. Each group is tailored to the specific social, intellectual and physical wellbeing needs of each groups.
Our activities include:
- Exercise and movement to music.
- Socialisation.
- Crafts.
- Games.
- Discussion groups.
- Outings.
- Special events.
- Guest speakers.
- Live entertainment.
- Therapies (for example music and art therapy).
Lunch, morning and afternoon tea and transport to and from the centre is included. Interpreters can be arranged.

Keeping Connected
Keeping Connected program is designed to make it easier for you to reconnect with social networks and activities in your area.
This program is designed for people who may have lost, or be at risk of losing connections to community. The Keeping Connected program worker will visit you in your home to hear about your social interests, hobbies and passions. They will listen to what is important to you and discuss options for outings or social groups. It really depends on your own unique interests and what you would like to try. We will support you in finding activities that are right for you, and can accompany you to your first sessions if needed.
Some options include:
- Visits to Senior Citizen’s Centres.
- Joining Neighbourhood Houses.
- Trips and gentle exercise classes at local leisure centres.
- Joining church groups.
- Regular lunch outings.

Cost: There is no cost for the work to support you attending activities, however you will need to pay for any activities or outings that you take part in.
Fresh Program
The Fresh Program provides fresh fruit and vegetables and social connections to people in East Reservoir.
Nutritious food and social connections are both important to people’s health and wellbeing. This group comes together to socialise and do activities, and go home with a fresh fruit and vegetables.
It is a great way to connect with people in the local community and make sure people have access to enough nutritious food.

When: Thursdays 10:30am-2:30pm.

Multicultural Social Groups
Who: Older people; people who have physical, functional, sensory, intellectual or psychiatric disability; people with acquired brain injury; carers and their families.
When: Mondays and Tuesdays, 9:30am – 2:45pm.
Where: Mondays at East Reservoir Health Centre; Tuesdays at East Reservoir Senior Citizen’s Centre.
Cost: Costs vary depending on your circumstances and outing location.
Italian Social Groups
Who: Italian speaking older people.
When: Mondays and Thursdays 9:30am – 2:45pm.
Where: Mondays at Preston (PANCH) Health Centre; Thursdays at East Reservoir Health Centre.

Memory-loss Social Group
Who: For people living with memory loss or dementia.

Kookaburra Club
‘Kooka’ is a place for Aboriginal friends and family to come together, have lunch, meet different people and be involved in social activities including outings. We also have regular guest speakers, support from Darebin Community Legal Service, information sharing and support from other services. Transport can be provided.
Who: People who identify as Aboriginal and/or Torres Strait Islander.
When: Mondays from 10am – 3pm.

Walking Group
