Pharmacotherapy Grant Announcement

Your Community Health is proud to announce it has been awarded a grant by the Victorian Department of Health to expand its pharmacotherapy services through an innovative, nurse-led model of care.

This funding will support the establishment of a new Addiction Medicine Nurse Practitioner-led service, designed to provide holistic, accessible and culturally safe support for people experiencing substance dependence. The model integrates the strengths of Your Community Health’s harm reduction and broader health and social services, offering culturally safe, wrap-around care. Leveraging the full expertise and scope of Addiction Medicine Nurse Practitioners to diagnose, prescribe pharmacological treatments, and monitor both physical and mental health, whilst coordinating care across health and community support systems.
“Our Nurse Practitioner led model of care improves access to bulk-billed pharmacotherapy and harm reduction services” said Cate Grindlay, Executive Director of Integrated & Primary Care. “We are working closely with our neighbouring community health organisations, community pharmacies, tertiary addiction medicine services, public health unit and primary health network to create a collaborative, responsive service and build local resilience in pharmacotherapy provision.”
