My Aged Care

My Aged Care participants can access a variety of services at our health centres or if appropriate, in your home.

These include:

  • Counselling
  • Podiatry
  • Physiotherapy
  • Physiotherapy exercise groups
  • Nutrition and Dietetics
  • Occupational Therapy
  • Restorative care
  • Social groups

How to access these services

If you wish to access our services, you will need to be referred via My Aged Care. Referrals can be made online or by calling on 1800 200 422. These can be made by your existing health professional or through a referral process by a non-health professional, such as a local council where you are receiving support services, a family member, carer or a self-referral.

When you contact My Aged Care, you can let them know that you wish to use services at Your Community Health.

Home Care Packages

You can access many of our services in your own home. If you are eligible for a Home Care Package through My Aged Care, just complete our referral form and someone will get in touch with you.

If you would like in-home support, but are not yet registered with My Aged Care you will need to contact My Aged Care online or by calling on 1800 200 422.

If you are having problems with your My Aged Care referral, contact us and we will help you.

About My Aged Care

My Aged Care logoMy Aged Care is a Government funding system designed to give you greater choice and easier access to aged care services. To access My Aged Care funded services you will need to meet eligibility criteria for the Commonwealth Home Support Program (CHSP). If you are not eligible for CHSP funding, you may still be able access these services through other publicly funded programs such as the Community Health Program.

Eligibility criteria for Commonwealth Home Support Program (CHSP)

There are different assessments depending on the services you would like to access. If any of the following apply to you, it is likely that you will be eligible:

  • Aged 65 years or older, still live at home and need help at home to continue to live independently, or
  • Aged 50 years or older and identify as an Aboriginal and/or Torres Strait Islander person, still live at home and need help at home to continue to live independently, or
  • Aged 50 years or older and on a low income, homeless or at risk of homelessness, and need help at home to continue to live independently.

An assessment by My Aged Care’s Regional Assessment Service will be able to confirm your eligibility.

If you are not eligible for My Aged Care

These services may still be available to you at our health centres. Contact us to book an appointment. A fee or waiting list may apply depending on your circumstances.
