All of our doctors are experienced General Practitioners, with individual special interests including Aboriginal and Torres Strait Islander Health, aged care, chronic conditions and pain management, diabetes management, mental health, paediatrics and children’s health, Refugee Health, Trans and Gender Diverse Health and women’s health.
Our services
Our services include diabetes management, care plans, aged care, Aboriginal and Torres Strait Islander Health care, women’s health, annual health assessments for those over 75 years of age, wound care, child and adult immunisations, minor surgical procedures, recall and reminders.
If you need to contact your doctor in an emergency, our staff will advise the doctor immediately. For general enquiries, our staff will take a message for your doctor.

From 3rd of November 2025, we are a bulk billing only clinic.
However due to capacity we are only able to accept new patients who:

Health Care Card and Pension Card holders and dependents

DVA card holders
-
Aboriginal and/or Torres Strait Islander people

Refugees and Asylum Seekers who have a Medicare card

Children under 16 years of age (fees will apply for Specialist Consultations for non-card holders)

Booking appointments related to the Pharmacotherapy program.
Our nurses
Our Nurses assist our doctors to provide a range of services to clients including wound management, immunisation, chronic disease management and care planning.
They can provide general health advice and education as well as managing the recall and reminder system for the practice
Health Assessments and Health Checks
If you are over 75, we recommend a yearly Health Assessment, which is a comprehensive review of a wide range of health issues. Clients aged 45-49 are also eligible and encouraged to have a detailed Health Check to help identify any chronic diseases you may be at risk for.
Our practice also provides Chronic Disease Management Plans, Home Medication Reviews and Mental Health Plans. Please speak with your doctor to confirm if you qualify. Medicare Australia has restrictions on who can/cannot receive management plans and when they are eligible
MyMedicare
It is voluntary and free to register in MyMedicare. When you register in MyMedicare, it notifies us that you have chosen Your Community Health as your regular care team. Registration gives you access to:
- greater continuity of care, including stable care planning, improving health outcomes
- longer Medicare Benefit Scheme (MBS) funded telehealth consultations with your GP
- triple bulk billing incentive for longer MBS telehealth consultations for children under 16, pensioners, and concession card holders

Interpreter Services
We offer free Translating and Interpreting Service and for all clients. Please let our reception staff know when you book your appointment if you need an interpreter. This free service is available 24 hours a day via telephone on 131 450. Visit their website for more information.
Another free interpreting service is available for patients who are deaf and use Australian sign language (AUSLAN). Please phone them on 1800 246 945 or visit their website for more information.
Regular health checks can help prevent you becoming unwell and relieve you of any concerns. We also offer a range of specialist medical services.
We give priority to people needing urgent medical attention. We accept walk-in patients, but you may have to wait, so we suggest booking an appointment online or calling us on 03 8470 1111.
After Hours Care
If you need urgent medical care and our clinic is closed, you can:
- Visit our Medicare Urgent Care Clinic Northcote at 42 Separation Street
- Call the National Home Doctor Service on 13 SICK (13 74 25).
In the event of a medical emergency, please call an ambulance on 000.
