We provide trusted and respected National Disability Insurance Scheme (NDIS) services to children aged 9 years and under, and to adults. We offer a range of therapeutic and social support services at our health centres and in the community.

We can support you to achieve the goals in your NDIS plan. We work alongside you, your support workers and the important people in your life to help you reach your goals and make the most of your abilities. 

Our NDIS services include Occupational Therapy, Physiotherapy, Podiatry, Nutrition and Dietetics, and Social Support Groups. These supports are delivered at our health centres and in community settings, depending on your needs.

The National Disability Insurance Scheme (NDIS) is the new way of funding and providing disability support. It gives you more choice in how and when you receive supports. People who are under 65 years old and have a permanent or significant disability may be eligible for NDIS. You can apply for an individual funded NDIS plan online or telephone 1800 800 110.

Our services are available to people in the North East Melbourne Area (NEMA) with an NDIS Plan. Complete our online referral form for adults, or you can also download the referral form and email it to us at ndis@yourch.org.au or call us for more information on (03) 8470 1111.

How to Get Started with NDIS Adult Services

1. Complete the referral form
Click the button below to fill in the online form. Alternatively, you can download the referral form and fill it out manually.

2. Submit your completed form
If the referral form has been downloaded and completed, email the form to ndis@yourch.org.au. A member of our team will be in touch to discuss the next steps.

3. We will contact you to confirm your referral
We will review your form and get in touch to confirm your eligibility and which services best meet your needs.

4. Start receiving support
Services will begin once your referral is processed and aligned with your NDIS plan.

Your NDIS Occupational Therapy Services for Adults

Occupational Therapy helps you participate in the everyday activities that are important to you. Together, we can help strengthen your abilities and help you adapt activities or the environment you live in to do everyday activities.

Some of the daily activities that we can support you with are:

  • Shower, dress and attend to personal care needs
  • Move around your house and community safely
  • Manage other daily and domestic tasks
  • Pursue hobbies and interests
  • Planning your days and routines.

We may do this by recommending specialised daily living equipment. We can also assess your home to determine if changes are required to help you do your daily activities. We can also help carers access information, resources or additional equipment.

Your NDIS Occupational Therapy Services for Children

We support children under 9 years old. We work in partnership with the family to support the following developmental areas:

  • Physical skills including fine and gross motor skills
  • Play and social skills
  • Sensory processing skills
  • Attention
  • Activity and emotional regulation
  • Self care skills including toileting, feeding, bathing and dressing

Your NDIS Speech Pathology Services for Children

We support children aged under 9 years old. We work in partnership with the family to support the following developmental areas: 

  • having difficulties following instructions 
  • not using words by 18 months of age 
  • not joining two words together by 2 years of age 
  • hard to understand by three years of age 
  • stuttering 
  • using a nasal, loud or hoarse voice.

Your NDIS Physiotherapy Services

We will work with you to improve your wellbeing, mobility and independence. Physiotherapists at Your Community Health will work with you to get the most out of your abilities and find solutions that work for you. We have experience working with people with a range of disabilities.

We can provide advice, support and therapy so you can:

  • Manage ongoing pain
  • Be more involved in physical activity
  • Improve your balance and strength so you can get around more safely
  • Work with your other health care providers, like podiatrists or occupational therapists.

Physiotherapy can include individual and group exercise programs, advice and education.

Your NDIS Nutrition and Dietetics Services

Your Community Health dietitians provide practical nutrition advice to foster wellbeing and function. The dietetics team at Your Community Health are Accredited Practising Dietitians (APDs), experts in nutrition. Our team has great experience working alongside community members who have a disability.

We can help you reach your NDIS goals, collaborating with you to:

  • Choose meals, snacks and drinks which foster daily fitness and function, so you can more easily participate in work, hobbies and tasks
  • Build skills and practical habits for activities such as meal planning, shopping and cooking
  • Achieve your health and wellbeing goals
  • Ensure your local environment and services accommodate your nutrition needs
  • Understand your learning and engagement style and provide nutritional advice considering this.

Our dietitians work with you and also your support network. We take the time to understand your needs and find solutions so you can meet your goals.

Your NDIS Podiatry Services

Podiatrists work with you to prevent, treat and manage foot, ankle and lower leg problems. Our Podiatrists can work with you to provide advice, equipment, treatment and so that you can keep as mobile, active and independent as possible.

We provide adult podiatry services to support your mobility and independence, including:

  • Nail and skin care for people unable to provide safe self-care
  • Assessment and treatment for foot and leg conditions
  • Foot posture testing
  • Footwear advice
  • Wound care and management of high risk foot conditions
  • Dry needling and foot mobilisation therapy
  • Foot assessments for circulation and sensation
  • Surgery and bracing for ingrown toenails
  •  Making referrals to other services and specialists, as needed.

Your Community Health is registered for prescription of Custom Foot Orthotics.

Your NDIS Social Support Groups

Attending one of our activity groups can help you to connect with other people in your community.
We offer a range of groups at our health centres, for people with similar interests and backgrounds.

This includes:

  • Italian social group
  • Kookaburra club (for Aboriginal and Torres Strait Islander people)
  • Ladies social group
  • Memory loss social group
  • Men’s Shed
  • Men’s social group
  • Mixed social groups
  • Multicultural social group
  • Digital learning group.

Our Activities
Some current activities include, exercise and dance, outings, celebrations and events, guest speakers, music and art therapy. Participants can get involved in planning their group’s activities.

Providing feedback

If you are a registered NDIS Participant and have a concern about your NDIS service, you can share your feedback in a few ways:

  1. Submit a feedback form with Your Community Health and we will investigate your concerns
  2. Contact NDIS to make a complaint online, by email or on 1800 800 110
  3. Contact the Health Complaints Commissioner, which responds to complaints about health services and the handling of health information in Victoria, online or on 1300 582 113.
