Physiotherapy

Our physiotherapy services will help keep you physically well and mobile to live the independent, active life you want. YourCH Physiotherapists have an advanced understanding of how the body moves and what keeps it from moving well.

Physiotherapy services are conveniently available to everyone at our three health centres.

Dietetics (food and nutrition)

We provide non-judgemental, practical food and nutrition advice to help you find and enjoy eating patterns that work for your needs.

The dietetics team at Your Community Health are all Accredited Practicing Dietitians (APDs). Learn more about our dietetics services today.

Exercise Groups and Classes

We offer a range of gentle exercise, fitness and strengthening classes at Your Community Health. From our ‘GLAD – Hip and Knee’ classes to the ‘Strong and Balanced’ group, all our sessions are facilitated by our physiotherapists and allied health assistants.

Group fees range between $5 and $8 per session for low and middle income clients, and more information can be found on our Fees Page for NDIS and Home Care Package participants.

Diabetes Education

We support people with diabetes to make important lifestyle changes that will help them look after their diabetes and reduce their risk of diabetes-related complications.

We offer both individual sessions to help support your specific needs, and group sessions for people wanting to connect with others with Type 1 or Type 2 diabetes.

Podiatry

We can help prevent and manage foot, ankle and leg conditions. Our services are available to everyone, from infants to older adults. From general foot care to specialist service, our team is available to help you improve your foot health.

Speech Pathology

Our Speech Pathologists work with young children to improve their communication skills.

We can provide assessments, individual and group therapy programs, home practice ideas and support for parents, secondary consultation (to Kinder teachers, doctors, maternal and child health nurses), and individualised programs for kindergartens with School Readiness funding.

Occupational Therapy (OT)

Occupational Therapy helps you participate in the everyday activities that are important to you. Our therapists will work with you to improve your abilities or help you to change the activities or the environment you live in to better suit your needs.

Our services are available for children and adults who have difficulties doing everyday activities that they need or want to do. We offer individual sessions, group sessions or home visits depending on your circumstances.

Optometry

The Australian College of Optometry offers eye health services at our East Reservoir health centre.

The clinic is independent from Your Community Health, but is conveniently located at our wulempuri barring health centre. Services include general services, children’s services and contact lens fittings.

NDIS

We provide trusted and respected National Disability Insurance Scheme (NDIS) services. We offer a range of therapeutic and social support services at our health centres and in community settings.

We can support you to achieve the goals in your NDIS plan. We will work alongside you, the key people in your life, and your your support workers to help you reach your goals and make the most of your abilities.

Aged Care

We offer a variety of aged care services and supports to help you feel your best as you get older.

If you are 65 years and older, or are an Aboriginal and/or Torres Strait Islander person who is 50 years and older, you can access aged care services via My Aged Care.
