Seniors Lifestyle Victoria is a free program supporting older people to stay active, strong, and connected.
About the Program
Seniors Lifestyle Victoria is a free program that helps older Victorians to stay active, stay strong and stay connected. The program helps participants to navigate the healthcare system so they can stay independent, healthy and well.
Support may include help with:
- Physical activity and nutrition
- Medication management
- Access to health professionals, including mental health support
- Social connection and wellbeing
A friendly coordinator will work with you to develop a personal plan that suits your needs and goals. Support is provided for up to six months, with a focus on building confidence and independence.
- People aged 65 years and over, or 50 years and over for Aboriginal and Torres Strait Islander people
- Living at home (not in residential aged care), including with a friend or family member
- Experiencing mild to moderate changes in health
- Living in Darebin or Banyule and able to attend in-person appointments

Who Seniors Lifestyle Victoria can help
This program is for people who would benefit from support including those who:

are finding it harder than before to get around or do everyday tasks such as shopping, cooking, cleaning or other activities they used to enjoy
-
have had a fall in the last 12 months

in the last six months have sought help for an unplanned health issue through Nurse-on-Call, an ambulance, an urgent care clinic, or hospital (including the emergency department)

can have multiple co-morbidities (medical issues) but must be clinically stable to remain at home
- are experiencing social isolation and/or loneliness

from culturally and linguistically diverse backgrounds and/or have limited verbal and written skills in English
- are at risk of poor health outcomes.
