Our program
Our program is a Victorian Government funded initiative that aims to increase health services across Victoria for trans, gender diverse and non-binary people aged 18 and over. This service is part of a consortium of Your Community Health, Ballarat Community Health, Thorne Harbour Health and Austin Health. All clinics oversee their own client intake and operations.
At Your Community Health, the services we offer as part of this program include specialist GP services and peer navigation.
Peer Navigation
What is peer navigation? ‘Peer’ means that our team are all members of the trans and gender diverse community. ‘Navigation’ means we can link people up with resources, healthcare, advocacy services, community spaces, and all sorts of other things!
Peer Navigation usually looks like submitting an access request form; being contacted by a peer navigator to answer a few questions; and then making time to have a chat about what your needs are at the moment, services you might like to be linked up to, and what’s available in your area.
- Matching you with a GP in your area who provides gender affirming care
- Finding mental health support and counselling
- Referral to social and peer support groups
- Access to dental and allied health services including physiotherapy, dietetics, podiatry and more.
As your first appointment with our service will be by phone with one of our Peer Navigators, we use a service access form to save you time.
Completing it takes less than 10 minutes and, once submitted, we will contact you shortly to complete intake and book your appointment.

Peer navigation is a free service. We also are pleased to offer other free or low-cost services to many clients across Your Community Health.
Fees for other services are based on income, or funding eligibility. See our Fees Policy for more information or ask us during your navigation session.
