An Allied Health Professional who assists clients in making lifestyle changes, to better manage living with a chronic health condition.

A Wellbeing Support Worker is an Allied Health Professional who supports clients with chronic conditions to improve self-management. They assist with care planning, referrals, service navigation, and health education, using motivational interviewing to tailor goals and build long-term wellbeing and independence.
- Assessing self-management skills, strengths, and gaps
- Developing /adding to Goal Directed Care plans
- Assisting with referrals, health service navigation, access to appropriate services, health information, encouraging effective communications by client, communicating with health team
- Health Coaching and Motivational Interviewing: Individual tailoring of goals to client’s values and needs, with motivational support
- Support development of long-term self-management skills

- Have at least one chronic health condition
- Non-acute presentations
- Self referrals welcome
- Exclusions: current exclusions for allied health services apply
