I am delighted that Your Community Health (previously called Darebin Community Health) is a part of Better Health North East Melbourne (BHNEM), a new collaborative platform.
This collaboration of service providers and organisations is focused on primary health care system partnership in order to improve health outcomes for people in Melbourne’s north eastern suburbs, including Banyule, Darebin and Nillumbik.
It will enable us to improve alignment between primary and secondary health care service providers in the region. The other BHNEM members are Banyule Community Health, healthAbility (formerly Nillumbik Community Health), Austin Health, Department of Health and Human Services, Eastern Melbourne Primary Health Network and North Western Melbourne Primary Health Network.
Working together with other agencies, we will be better equipped to support the management of chronic illness, including chronic mental illness, for people at risk of poor health outcomes.
This will help to reduce avoidable hospital admissions through improved community-based models of care.
