NDIS

Disability social group

We provide trusted and respected National Disability Insurance Scheme (NDIS) services.

We can support you to achieve the goals in your NDIS plan.  We will work alongside you, the key people in your life, and your your support workers to help you reach your goals and make the most of your abilities.

We offer a range of therapeutic and social support services at our health centres and in community settings. Any NDIS Participant can access the following NDIS-funded services at our health centres:

  • Physiotherapy
  • Occupational Therapy
  • Dietetics & Nutrition
  • Podiatry
  • Group and centre-based activities

Start using these services

Our services are available to people in the North East Melbourne Area (NEMA) with an NDIS Plan.

About the NDIS

The National Disability Insurance Scheme (NDIS) is the new way of funding and providing disability support. It gives you more choice in how and when you receive supports. People who are under 65 years old and have a permanent or significant disability may be eligible for NDIS.  You can apply for an individual funded NDIS plan online or telephone 1800 800 110.

You can request Your Community Health services when submitting your application.

If you would like to discuss how we can assist you through the transition to the NDIS, contact us on (03) 8470 1111 or [email protected], or visit our East Reservoir Health Centre. You do not need an appointment.

If you are not eligible for NDIS:

The Government has agreed to make sure you can continue to get the services you need. Let us know if you can’t get NDIS support and we can talk to you about your options and how we can help.
