New community connection service during COVID-19

You will have heard that as a result of the COVID-19 pandemic, we have temporarily stopped running our social support groups. But did you know that we are still reaching out to our clients and community members?

Our priority is to help our clients and community stay healthy, safe, connected in this challenging time.  So we are regularly contacting community members to provide care coordination and supports whilst they are in self isolation.

I’m sure you will have experienced the disruption that the COVID-19 pandemic has caused. For many of our clients and community members it has limited their ability to connect with other people and access health and other services. For people who rely on our services for social connection, it is important that they don’t feel completely alone during this time and also that they get the extra personal and resource support they need to get through this.

So we have redeployed some staff members as Care Coordinators and the Support and Outreach workers., who will contact people in the community and help identify their needs and refer appropriately internally as well as externally.  The team is aligning with the City of Darebin’s response, including emergency relief services, to ensure maximum benefit.

Some of the things we can do include:

  • A weekly phone call to chat and stay connected
  • Ensure people have the food and medication they need at home
  • Connect people with others
  • Provide tips to stay healthy at home
  • Connect people to other health and community services

We are already contacting clients, sharing this information with local agencies working in the area and dropping off leaflets in the local area. But we can also take self-referrals or introductions from friends and family members. Feel free to get in touch.
